The NTSB concluded that configuration and programming errors, incorrect positioning and heading data, inadequate supervisory oversight, and the absence of collision-avoidance technology caused dozens of Sky Elements drones to collide during a December 2024 holiday show at Lake Eola in Orlando. Several drones entered the audience, seriously injuring a seven-year-old spectator, while at least 47 aircraft were reported to have collided or fallen. The incident prompted operational changes at Sky Elements, including two-pilot operations, defined responsibilities, and signed preflight checklists, and also led to litigation involving the operator and other parties.
